Let VISUAL EVIDENCE / E-DISCOVERY LLC (VE) do the work for you. You make the call…we’ll gather the Patient Health Information (PHI), prepare it for review, and provide easy access for document viewing.

VeREPOSITORY is the secure, on-line, repository service established by Visual Evidence for requested PHI. Upon approval and qualification by the initiating attorney, Visual Evidence requests the patient information from the medical facility, scans the received documents, applies Bates labels to them, and uploads the final documents to a secure internet site for review.

Medical Records Repository Services and Process

• Records identified by the medical facility will be forwarded to the offices of VE
• VE will Bates stamp each of the provided records
• VE will create an electronic version of each of the records
• VE will post all records to VeREPOSITORY, a web-enabled review platform, for designated Plaintiff counsel to review
• VE will provide Plaintiff’s counsel with a secure log-in and password for VeREPOSITORY
• Plaintiff’s counsel will have ten (10) days from the time of notification to review the records
• Plaintiff’s counsel will be provided the ability to “tag” documents that are believed to be privileged
• Upon expiration of the ten (10) day time frame, VE will forward a PDF version of all documents to Plaintiff’s counsel except for those “tagged” by Plaintiff’s counsel as being privileged
• Documents tagged privileged will be provided to the designated Court in PDF format for in camera inspection
• Please note that X-rays, MRIs and CT scans cannot be sent electronically and therefore fall outside the scope of our primary repository services. Attorneys may schedule review of these types of records at Visual Evidence / E-Discovery in person
• Associated costs include pick-up and delivery associated with each engagement and $.25 per page for records loaded to the review site
